2. Information We Collect
We collect the following information about you:
Contact details — your name, phone number, email address, and home address;
Date of birth;
Information you give us on your Eligibility Call, including your treatment goals and the package you select;
Payment and financial information, including details necessary to set up and administer a payment plan with us;
Your dental scan or impressions, used to create your Smile Design Preview;
Photographs or videos, where you have separately consented to these being taken and used;
Limited clinical information shared with us by your Treating Practice — specifically, confirmation that your Clinical Assessment and Treatment have taken place, and their outcome. We do not receive or hold your full clinical records.
3. How We Use Your Information
We use your information to:
Arrange and administer your Composite Bonding Treatment and/or Whitening Treatment, including referring you to a Treating Practice;
Create your Smile Design Preview;
Set up and administer any payment plan you choose, including taking payments and contacting you if a payment is missed;
Communicate with you about your appointments and treatment journey;
Comply with our legal and regulatory obligations; and
With your separate consent, use photographs or videos of your treatment for marketing and educational purposes.
4. Special Category (Health) Information
Some of the information we hold about you, including information about your treatment, is “special category data” under UK GDPR, which receives extra legal protection. Bondify relies on your explicit consent as the lawful basis for processing this information, which you provide when you sign our Patient Treatment & Payment Agreement. You may withdraw this consent at any time by contacting us, although this may affect our ability to continue arranging your treatment or payment plan.
5. Who We Share Your Information With
We share your information with:
Your Treating Practice, so that they can contact you, carry out your Clinical Assessment, and deliver your treatment;
The independent smile designer contracted by Bondify, so that your Smile Design Preview can be created;
Our laboratory, for the production of any smile trial mock-up or appliance needed for your treatment;
Payment processing and finance administration providers, to administer your payment or payment plan;
IT and case management system providers who support our booking and administrative systems.
We do not sell your personal information to any third party.
6. Your Treating Practice’s Use of Your Information
Bondify and your Treating Practice each act as an independent data controller for the information they hold about you, for their own respective purposes — Bondify for the commercial and payment relationship, and your Treating Practice for your clinical care. Your Treating Practice has its own privacy notice covering how it handles your clinical records, which you should ask for separately if you would like more detail.
7. International Transfers
Bondify does not transfer your personal information outside the United Kingdom.
8. How Long We Keep Your Information
We keep your information for as long as you have an active relationship with us, including any ongoing maintenance plan or pay-as-you-go maintenance arrangement following your treatment. Once that relationship ends, we keep your information for a further 7 years, to allow us to deal with any legal, contractual, or financial matters that may arise, after which it is securely deleted.
9. Your Rights
Under UK GDPR, you have the right to:
Access the information we hold about you;
Ask us to correct inaccurate information;
Ask us to delete your information, in certain circumstances;
Ask us to restrict how we use your information;
Object to certain uses of your information, including direct marketing;
Receive a copy of your information in a portable format; and
Withdraw consent at any time, where we rely on consent.
To exercise any of these rights, please contact us using the details in Section 1. You also have the right to complain to the Information Commissioner’s Office (ico.org.uk) at any time.
10. Cookies and Website Use
We use cookies on our website for the following purposes:
Essential cookies, which recognise you as a returning visitor and remember information you have already given us, so that if you fill in one form and later complete another, your details are carried across rather than needing to be entered again;
Analytics cookies, where used, which help us understand how visitors use our website so we can improve it; and
Marketing cookies, where used, which help us understand the effectiveness of our marketing.
You can control or delete cookies through your browser settings at any time. Please note that blocking essential cookies may mean some parts of our website, such as carrying your details between forms, do not work as intended.
11. Children
Our services are not intended for, and we do not knowingly collect information from, individuals under the age of 18. Whitening Treatment specifically is not available to patients under 18.
12. Security
We take appropriate technical and organisational measures to protect your information against unauthorised access, loss, or misuse.
